Unknown Control panel/Toolbar

Hello,

Yesterday this image showed up on my screen while I was browsing using Firefox v28.0.

I have never seen it before and I have no idea how it got there, what it is or how I got rid of it! I don't know which web site I was on when this happened.

Can anyone explain this to me, please?
